Course Overview
Complex Care Management Rounds exposes community-based primary care providers(Physicians, nurses, PAs) adn social workers to integrated, longitudinal, and interdisciplinary care through a panel of patients with high complexity of health and social needs from within the community. Through case discussion, CCMR will improve the health outcomes of patients with complex needs. These interdisciplinary rounds will also focus on understanding patients’ lived experiences and the impact on their health outcomes and experience with the health care delivery. Understanding patients with complex needs, who are often marginalized and experience bias in the delivery of health care, supports improved systems based practice. CCMR occur through twice a month online rounds. Participants are expected to present a patient case twice each year.

Learning Objectives

After completing this activity, participants should be able to:


  1. Discuss complex patient needs and facilitate interdisciplinary care rounds for patient care management and peer consultation
  2. Discuss how to demonstrate the capacity to respect patients and team members during the case review process
  3. Identify the impact of social determinants of health on patients' health outcomes
  4. Identify biases inherent in the transfer of patient information from one team member to another
  5. Describe opportunities to coordinate care between organizations or institutions

Accreditation

In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.  

Designation of Credit
Physicians: Penn Medicine designates this live activity for a maximum of 1.50 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.50 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Physician Assistants: AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by ACCME or a recognized state medical society. PAs may receive a maximum of 1.50 Category 1 credits for completing this activity.

Nurses: This program provides 1.50 NCPD Hours.

ABS MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn 1.50 credits toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
