Pulmonary hypertension (PH) and pulmonary fibrosis/interstitial lung disease (ILD) are individually complex, progressive diseases with substantial morbidity and mortality. Pulmonary vascular dysfunction and parenchymal lung disease individually pose complex challenges to management. Increasingly, it is recognized that these diseases cannot be considered in isolation and frequently co-occur. The emergence of novel effective therapies in both diseases (including in combination) makes diagnosis, prognosis, treatment selection, and clinical trial design challenging.

The 2027 Alfred P. Fishman Symposium will address this rapidly evolving area. The proposed program brings together experts in ILD, pulmonary vascular disease, physiology, imaging, immunology, transplantation, and clinical trials to address an important unmet need: how to identify the biological and clinical phenotypes of patients with PF, PH, and PF-PH and translate that information into more appropriate treatment. The agenda specifically addresses ILD and PH diagnostics, evidence-based treatment, physiology, mechanisms of epithelial-endothelial and vascular-parenchymal crosstalk, clinical phenotyping, and challenging treatment decisions.

Specialties - INTERNAL MEDICINE - Pulmonary Disease
Professions - Industry, Nurse Practitioners, Pharmacists, Physician Assistants, Physicians, Registered Nurses, Students

Objectives

After completing this activity, participants should be able to:

  1. Apply diagnostic and treatment algorithms for ILD, PAH, and ILD-PH, incorporating current evidence-based and emerging therapies.
  2. Explain the pathobiologic crosstalk between pulmonary endothelium and epithelium and its relevance in ILD and PH.
  3. Formulate individualized treatment plans across ILD, CTD-PAH, ILD-PH, PAH, and sarcoid-PH phenotypes using a case-based approach.
  4. Evaluate emerging clinical trial designs aimed at improving PH/ILD research.
  5. Describe current regulatory perspectives on endpoints and regulatory pathways for PH and ILD, and discuss the ethical and methodological challenges of trial design in rare pulmonary disease.
