Course Overview
The primary focus of the Primary Care Training Enhancement (PCTE) program is to train primary care clinicians to lead health care transformation and enhance community-based teaching to promote better primary care practice within underserved urban areas. The Complex Care Management Rounds (CCMR) is an interdisciplinary case review process for primary care providers who share complex patients. The purposes of the CCMR are to expose community-based primary care providers to integrated, longitudinal, and interdisciplinary care through a panel of patients with high complexity of health and social needs from within the community and improve the health outcomes of patients using a case conference review process. These interdisciplinary rounds will also focus on understanding patients’ lived experiences and the impact on their health outcomes and experience with the health care delivery, particularly, understanding that patients with complex needs, who are often marginalized experience bias in the delivery of health care.

Specialties - EMERGENCY MEDICINE, FAMILY MEDICINE, INTERNAL MEDICINE, OBSTETRICS AND GYNECOLOGY, PEDIATRICS, PSYCHIATRY


Objectives
  1. Participate in IDT meetings for both patient care management and peer-consultation
  2. Facilitate IDT meetings for patient care management and peer-consultation
  3. Identify the impact of social determinants of health on patients' health outcomes
  4. Demonstrate the capacity to respect patients and team members during the case review process
  5. Identify biases inherent in the transfer of patient information from one team member to another
  6. Discuss health information exchange clinical repository to access information related to the care of their patients

Accreditation

In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
