The Anesthesiology and Critical Grand Rounds consists of a weekly lecture series featuring speakers from a variety of academic institutions as well as Penn that covers a range of topics germane to the field of anesthesiology. The goal of this series is to improve the participants’ medical knowledge in the realms of preoperative, intraoperative and postoperative anesthetic care, review and integrate evidence-based advances in the field of anesthesiology and its subspecialties, and provide attendees a forum for the discussion of current clinical and basic science research with recognized leaders in the field.

Objective(s): 1 Identify system and human factors that contribute to wrong-site surgery and categorize them using an RCA framework 1 Describe effective interprofessional communication strategies.  2 Discuss the perioperative workflow to identify where safeguards failed or were absent in the mock case. 3 Discuss how to develop actionable, system-level recommendations  that address root causes identified in the mock RCA, including at least one change to process/policy and one change to team practice.
