The Population Health in Primary Care Grand Rounds Series focuses on the integration of population health into primary care practices, aiming to enhance health outcomes at both individual and community levels. Participants will explore and learn about population-based approaches to addressing the social determinants of health, implementing preventative strategies, and managing chronic disease. This series is accredited for physicians, nurses, nurse practitioners, physician assistants, and pharmacists.

  2. Describe population health needs and chronic disease management
  3. Describe health and disease prevention strategies
  4. Discuss cultural competencies and enhance health equity and SDOH
