Course Overview
Department of Medicine faculty and trainees review cases where morbidity and/or mortality occurred. These include all "Below Expected Risk" deaths and cases that highlight important clinical and system topics. Lead by the PPMC's Patient Safety Officer, Hospitalist leadership and the Chief Resident, cases are presented critical audience with a goal of minimizing preventable death and morbidity.

Learning Objectives

After completing this activity, participants should be able to:


  1. Discuss the utility of Patient Safety Net, which cases to file, what information to include and have confidence in the system’s response to entries.
  2. Discuss the Root Cause Analyses process, especially how countermeasures are developed and how the department plans to implement them.
  3. Discuss safety science and HRO principles to employ daily in their work (i.e., Just Culture) but also describe difficult cases encountered by colleagues.

Accreditation

In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.  

Designation of Credit
Physicians: Penn Medicine designates this live activity for a maximum of 1.00 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.00 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Physician Assistants: AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by ACCME or a recognized state medical society. PAs may receive a maximum of 1.00 Category 1 credits for completing this activity.

Nurses: This program provides 1.00 NCPD Hours.

ABS MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn 1.00 credits toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
