To provide evidence-based interprofessional education about Palliative and End-of-Life care to Penn Medicine hospital-based, outpatient, and home care clinicians who engage in the care of patient with serious illnesses.

Objectives

After completing this activity, participants should be able to:


  1. Discuss recent palliative care research and describe how findings apply to local clinical practice.
  2. Discuss evolving approaches to improve serious illness communication.
  3. Describe holistic practices to provide psychosocial and spiritual support to patients and caregivers.
  4. Describe strategies for ensuring safe transitions of care for across the health care continuum for patients facing serious illness.


In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
