This educational activity is based on a recording of a live webinar in the Geriatrics for Practicing Professional Series that took place at the Perelman School of Medicine at the University of Pennsylvania on May 24th, 2017. An interdisciplinary presentation involving a geriatric physician, a palliative medicine physician, a social worker, and a chaplain introduces the learner to some tools to determine prognosis, how to communicate that prognosis effectively, and how to support the patient and family after the communication. 


Maintenance of Certification (MOC) Successful completion of this CME activity which includes participation in the evaluation component enables the participant to earn up to 1.00 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC points.

Learning Objectives

  1. Utilize basic tools to assist in the determination of prognosis in long-term illness
  2. Develop a framework for a plan to communicate prognosis to patients and families
  3. Describe the emotional and spiritual impact of communication of prognosis
  4. Employ appropriate support and follow-up after conversations regarding prognosis take place
