Course Overview
In this CME Activity, participants including community and academically based primary care providers in multiple disciplines will attend bi-monthly didactic sessions. These sessions will bring content experts from many aspects of the University including Penn Medicine and CHOP to address relevant and contemporary issues in delivering care to medically underserved patient populations. Participants will utilize an online program webpage for reference materials and pre-session discussion boards. Participants will apply and report back on the application of content to their own practice setting and work. All participants will complete evaluations of each element of the CME series.

Target Audience
Specialties - FAMILY MEDICINE, INTERNAL MEDICINE, PEDIATRICS, PEDIATRICS - Adolescent Medicine, PUBLIC HEALTH AND GENERAL PREVENTIVE MEDICINE

Learning Objectives

After completing this activity, participants should be able to:


  1. Learn rapid cycle quality improvement (CQI) initiatives
  2. Review evidence-based interventions for complex care patients
  3. Identify components of nterprofessional team-based care
  4. Assess network opportunities, develop ways to expand and manage network opportunities, and identify ways to work within the system to harness power to achieve health outcomes
  5. Define principles of interorganizational collaborations and discuss ways to develop respectful academic-community partnerships
  6. Demonstrate an understanding of the processes involved in leadership development on individual and organizational levels, and how these processes must be sensitive to the needs of the population
  7. Develop personal goals and a vision statement for the future as a leader in health care of disadvantaged and marginalized communities.

Accreditation

In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.  

Designation of Credit
Physicians: Penn Medicine designates this live activity for a maximum of 1.50 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.50 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Physician Assistants: AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1 Credit™ from organizations accredited by ACCME or a recognized state medical society. PAs may receive a maximum of 1.50 Category 1 credits for completing this activity.

Nurses: This program provides 1.50 NCPD Hours.

ABS MOC: Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn 1.50 credits toward the CME and Self-Assessment requirements of the American Board of Surgery’s Continuous Certification program. It is the CME activity provider's responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
